FOR RETURNING PATIENTS ONLY, NOT FIRST TIME VISITS. This is for patients/clients who have already received your initial physical therapy evaluation. Now it's time to begin your road to recovery with your EAPT family. Here is where we make a commitment to stick to your personalized treatment program designed by your Doctor of Physical Therapy until you are able to confidently leave our facility feeling even better than when you came.

Cancellation Policy

Cancellation fees are $50 if cancelled less than 24 hours prior to appointment. $75 no show fee. To reschedule, please contact us at frontdesk@elitealliancept.com.
